No play back on 1 room DVR.

When trying to watch anything recorded on DVR, it freezes and will not fast forward.  This is only on 1 DVR.  TV is working fine

The symptoms you describe occur when there's a signal issue.  Live TV may work normally, but remote commands may fail.  When you press a remote button for a cable box function (pause, reverse, FF, etc.), the box sends a command to a Cox server which sends back a reply.  Playback will freeze if the reply from the server is never received.  This would also apply to live TV when selecting pause, reverse and/or FF).  TV commands (on, off, volume change) are unaffected.

Live TV ff and rewind work fine and streaming works fine. Just issue with DVR recording playback

It could be a defective DVR.  It would require a bit of effort, but before swapping it and losing all of your recordings, you could try testing it at a different cable outlet.
